What should I do when I'm on the verge of confronting a fear and panic sets in?

Hi, I have panic attacks every time I'm going to face one of my fears, be it public speaking, medical procedures, or even answering the phone. My body tenses, my heart pounds, and I'm sure I'm going to pass out. I try breathing and remaining calm, but the terror wins out. I want to know what really works in that split second when you're facing something dreadful and your mind begins racing. Is there a way to ground myself and just go on without letting panic get the better of me?
