How do I cope with anxiety when I'm confronted with something truly challenging?

Any time I have to accomplish something difficult such as give a talk or make an important decision my anxiety goes through the roof. I get lightheaded, my heart pounds, and I just know I'm going to screw everything up. And even if I rehearse, the fear doesn't dissipate. I'd like to be able to tackle difficult assignments without disintegrating. How do I deal with severe anxiety when I'm faced with something that just feels too big?
